I wanted to take a moment to check in with you, because this community exists for you. I’ve been reading through exit surveys and noticed some folks mention they were hoping for more of my presence here. I really appreciate the honesty. It helps me grow and serve you better. ❤️

Just to give a little behind-the-scenes: I post here daily (Monday through Friday, sometimes Saturdays too), share 2–4 videos a month, and host regular livestreams, like our book club, insider hangouts, and guest interviews. Do you have any tips for safely grooming pubic hair? I have excessive body hair and prefer to keep things nice and neat downstairs, but I sometimes struggle when shaving the delicate wrinkly skin of my scrotum.

Dr. Susie I had watched your video "pump like a pro" but I was more interested in how to use a penis pump just for penile rehabilitation purposes. Would I use the same the same procedure for rehab purposes? I need penile rehab because I do not get spontaneous erections any more. Dr. Susie, I am about to start using a vacuum erection device but I'm a bit scared that I might cause an injury to the penis. Can you give me your recommendation on how often to use the device and how long for each session. Thank you Dr. Susie.

Yes, that’s definitely a possibility in both cases. Medications like PDE5 inhibitors or injectable therapies may be the most helpful here. I’m also a big advocate for cardiovascular exercise, it’s well-supported in the research for improving erectile function.

Dr. Susie, I hate to keep asking questions but I trust you more than my urologists for these types of questions. So I am 66 years old and I turned diabetic 5 years ago when they removed most of my pancrease due to pancreatic cancer. Since then I have been able to maintain an A1C of 6.4 which according to my endocrinologist is very good. So even if I have controlled my A1C, am I still going to have erectile issues just by virtue of the fact that I have diabetes? I also have a history of hypertension but has been very well controlled with medication, so same question am I going to have erectile issues just by virtue of the fact that I have hypertension?

I’m sorry to hear your first pelvic therapy visit didn’t feel supportive. Loss of spontaneous erections can have several causes, like cardiovascular issues and diabetes are two common ones that a medical doctor would assess. From a pelvic health lens, your therapist should ask specific questions about your sexual function to help figure out if a referral to a medical doctor and/or sex therapist makes sense. A thorough pelvic exam would look at things like sensation, muscle tone, strength, and coordination.

Dr. Susie, I recently posted a comment on a bad experience with my first pelvic floor evaluation/consultation. I have found another place where the initial consultation was more on education and my next visit will be the external/internal exam. One of my main issues that I want addressed is that I have lost spontaneous erections (night time, morning and day time). Is there a specific therapy or treatment procedure that I should ask for to fix this issue? Please give me your thoughts.

Ideally, I'd love to have real live model for the treatment techniques but having a difficult time finding a model with intact foreskin. Tight foreskin may be considered phimosis-- there are varying degrees of tightness but in general, phimosis is when the foreskin is unable to be pulled back over the glans.

Yes, it's possible to have pelvic floor muscles that are "weak" because of diminished ability to voluntarily relax. This can change the power produced if the range of motion of the muscle is limited. Symptoms are just one piece of the picture and an external and internal examination is necessary to confirm.

One more thing Dr. Susie, that therapist said that I have both weak and tight pelvic floor muscles just based on my the symptoms that I described to her. Is it possible to have both?

Thanks for sharing your first session experience with us, CS. Sounds like it was an incomplete assessment, especially for an initial evaluation. A quick convo and one cue-based exercise like “holding in poop” without any palpation or internal assessment doesn’t give the full picture of what’s going on. It’s frustrating when care feels like a checkbox. You deserve a provider who takes the time to really understand your body and symptoms. Let me know if you want help finding someone else in the Houston area.

Dr. Susie, I just wanted to share my first pelvic floor therapy experience yesterday. I saw a therapist with one of the big urology clinics in Houston. The initial consultation lasted about 45 minutes where we talked about my symptoms (this in normal for any initial consultation. The last ten minutes she showed me an exercise that I would do at home, she said like "holding my poop" which I guess was some form of a kegel. All this was done fully clothed so no palpation of the perinium nor an internal exam. She then sent me off and said we will follow up in one month. NOTHING like one of your initial evaluations.

The honest, frank, (not too serious) uncensored tutorials that truly get to the point of helping and recognizing that we’re wanting relief from the pain and discomfort, and not caught up showing us on a photo, diagram, or plastic model have been nice. Just being able to see how to do this stuff ourselves, even if we are already seeing a pelvic floor physical therapist in person, it’s the alternate opinion/take/technique or even hearing an opinion/technique that might arise again that reinforces or might help find relief from the discomfort.

I was recently referred to PTF and this has helped relieve a lot of my anxiety about what it all entails to go for this type of therapy (I didn't even know it existed). The one topic I haven't seen anywhere is how this therapy can specifically help with fecal incontinence, as that is one of the reasons I was referred. Thanks for all your care and concern!

I originally came here because I was new to pelvic floor physiotherapy, and was trying to learn and build expectations. I'm mostly staying for curiosity satisfaction, especially related to information and content that doesn't really exist elsewhere. I've been taught my whole life, my testes and penis are a "problem to be kept silent about". This is the first place where I feel like my organs are allowed to be part of me.

The frank and sometimes funny discussions help remove some of the stigma and mystery surrounding this aspect of men's health. I for one am glad I found this community and plan to stay a long time. I hear you, Douglas—this one’s tricky, especially with massage therapy given the varying state laws and scope of practice acts. I do know a few massage therapists who venture into what some might call “grey” areas, but they’re definitely the exception, not the rule.

I think that you should lead a coalition to refer patients to therapy that could not be done under the existing laws. A pelvic floor doctor should be able to write a Rx for pelvic floor massage therapy. Massage therapist should be trained and licensed to do these. It would legitimize the medical need.

There are not many resources out there for men like you offer and that's something to be proud about! Your videos on pelvic therapy for men helped me peel back layers of inhibition and try to feel at ease (normalize?) taking care of my health.
