To join Discord go to Patreon profile settings > Apps > Connect Discord.

Make sure your Discord account is not restricted or that you haven't reached the 100-servers limit.
If you are new to Discord, you may need to verify your email.
Ensure that your Discord account is up to date in your Patreon profile (e.g.: if you've recently changed your nickname).
Some web browsers like "Floorp" have been reported as not working to connect Patreon with Discord (try a different one).
