Its mostly due to the sheer size of the Gag, seriously the thing takes up almost half of Trish's face. It looks like the sort of things that would be strait up painful to have, kind of like third leg penises in a lot of porn except its in her jaw instead of the theoretically stretchy vagina. Still looks great other than that.
