Another thing I think you sort of misunderstood about this episode, in a sort of correct way, is what was meant by Kaguya/Miyuki not being able to confess if Ishigami were rejected. Miyuki wasn't worrying from the perspective of them being worried a confession would get rejected because they saw an example of it getting rejected, rather he was saying that to confess right after one of their closest friends got rejected would just feel like a bad move, like Ishigami would be made to feel even worse seeing them find love when he failed. Your interpretation is another reason the confessions might not occur, but not what Miyuki was thinking.

Ishigami's faults that Chika wrote down on the paper were the following:
-He's narrow minded and quick to find faults with others.
-He's always ready to verbally abuse me
-When that jerk hurts my feelings, it takes a long time to get over it.
-He has no respect for upperclassmen
-Sometimes he genuinely looks down on me.

The manga was much more clear about the Ishigami confession thing. It stressed how 10% of students don't know about the significance of the heart and makes it seem like Ishigami made a ballsy confession but he really just thought she wanted a cookie. It even explains that because he was generally a loner for much of his time at Shuchin he wouldn't know since it is one of those word of mouth stories.

That part was so funny in the manga. Ishigami just unknowingly confessing his love to Tsubame in front of her classmates. He just went out of his way to embarrass her without realizing it.

Ishigami had no idea he just confessed his love. As the episode said, 10% of people have no idea about the heart legend. So he just confessed his love without realizing that he did so. He just got her a cookie that she liked. Not only that, but he also did it right in front of her class so Tsubame pretty much got confessed to in front of all her classmates which raised her embarrassment. Ishigami is still completely unaware that she is thinking about whether to accept his confession or not.

Just so ya know, the manga is WAYYYY clearer, but Ishigami doesn't realize he confessed. He merely got the cookie because Tsubame wanted it. That is kinda important but for some reason they didn't emphasize the fact he didn't realize too much.
