Ok catching up on previous topics - sorry to be so late to the party!! You took a silent retreat?? WOW!! How long did it last?? Did you have to be silent during the entire retreat?? I am Catholic and many, MANY years ago was a seminarian for 6 years (4 years in HS and 2 more in college before transferring to a midwestern urban Catholic Jesuit university). When someone is going through the steps to be ordained a Catholic Priest, the final step before going to Divinity School is a yearlong meditative deep prayer process called Novitiate. During that time there is a month-long SILENT retreat where the Novitiate candidates have to remain completely silent for an entire month in prayer. Whenever I hear of a silent retreat, I think of that process...... I don't know if I could keep my mouth shut for that long - I am a former DJ and have a degree in broadcasting/electronic communication so being silent for that long would really be EXTREMELY difficult

Oh! I talked to my PT this morning about what happened after the self massage. She thought this group was super interesting! This is the first time she was willing to attempt direct intervention where I get this super painful knot. Thanks for making these videos, it helped me bridge a treatment gap by explaining how I got to a working theory based on SCIENCE!

Hi Rex! I completely understand your frustrations. It’s unfortunate that more pelvic therapists aren’t as comfortable discussing sex with men. It’s ironic that questionnaires ask about sexual health, but clinicians don’t address these topics. (Sigh).

Dr. G. So happy to hear your perky attitude again, like the "when your crotch bothers you video" I really missed that. I've had 2 - PFPT sessions not sure what to think. As a DPT thought they would be easier to talk to but they just don't have your passion for the sexual aspect of the pelvic floor function, which is strange because the first questions on the Male Pelvic Floor Dysfunction Form is all about sexual function, ejaculation and genital pain. I'm ready to learn from you, teach me some more please! Rex

The sensations can be dampened or amplified by our emotional and mental state…conscious and non conscious… wow so deep! This is a whole topic in itself. When I was actively modeling non de for the arts, not only was it to collaborate and be my extension of art it was a meditative state. I was deep into meditation practices and it’s a whole thing to be on stage for a group and at the same time be somewhere else I. Your own mind.

If I do too many pelvic floor stretches in one sitting after not having done it in a long time, the ares gets real irritated like I'm having a flare up. So have to be mindful not to push too much too quickly.

My theory is it is the bulbo muscle that is super tight and not something we have treating at my PT. I'm going to talk with them on this week's visit.

I very sincerely wish I could just have an academic attitude. Sometimes when I'm anticipating treatment I visualize that I can and then the next second it's all panic again. I came pretty close to this on about the second or third session where I became curious and asked the therapist to help me locate the muscles. Probably the most relaxed I've been as the two of us took turns tracing out locations. This changed radically when I asked her to trace the ischiocavernosus from one end to the other. THAT was overwhelmingly triggering and I had to ask her to stop.

Appreciate the warm welcome, Robert. This is the first time I've been online in 2 weeks! This picture was taken sometime last year and the puppet was given to me as a gift by one of my professional mentee. It's my understanding that the person who hand crochets these puppets is no longer making them due to health reasons. I wish them well and glad I was able to get this precious gift before they closed shop. The foreskin moves up and down and the head has eyes and a smiley face, haha.

Hiya, Ned! Thanks for the warm welcome. As you can imagine, I have many follow up questions related to your experience of the flare up after exploring with the massage. Unfortunately, this isn't the space I can dive into those details as a medical professional. I can say that flare ups can feel alarming and it doesn't always mean hurt or harm—it could mean that there was too much stimulation all at once activating nerve sensors related to various types of stimuli like caressing touch, light touch, firm touch, temperature, vibration, pressure, etc. If pain has persisted for a long time, these sensors get really good at protection—even when there is no damage or harm or need for protection. All that to say, if you are working with a local pelvic therapist, I suggest brainstorming ways you can explore with touch in this area that hurt less or feel good which may mean touch areas outside the perineal area and working towards as you build trust and confidence in feeling safe.

Hey Guy, Thanks for sharing! The ischiocavernosus muscles are located on either side of the area where the inner thigh meets the pelvis—called the ischiopubic rami. They are tiny muscles that sit just below the surface of the skin and over the erectile tissue (corpora cavernosa). It is very diffcult to completely isolate these muscles as they work in synergistically with the other pelvic floor muscles, like the bulbospongiosus. A rough idea is good enough 🙂. Yes, there are can be many "yummy" sensations felt in the pelvic area, especially when coupled with an overall pleasant and comfortable explorative experience. The physical sensations we feel in our body can also be amplified or dampened by our emotional and mental state which influences the complex symphony of physiological events both conscious and non-conscious. No, the model in the video was not sedated. 😅 I know that this person intentionally practices to maintain a peaceful and relaxed mind with various situations—maybe this is why they are able to remain relaxed during a sensitive exam while being recorded. 🫠

Still working on identifying the ischiocavernosus. I'll see if my wife can help with that. The bulbospongiosus is a no brainer. Since becoming aware of my pelvic muscles I'm aware of all sorts of very pleasant (yummy?) sensations. Trying to figure when and how they are triggered and whether some are the ischiocavernosus in action. When my wife saw the video on location of muscles, her first comment was "Is he sedated?". Curious minds want to know. Personally, I'm incredibly envious of someone being able to remain relaxed and would like to desensitize to that level.

Welcome back! I tried the massage and it caused a flare up. I was able clearly feel a difference between the left and right side of a specific pelvic floor muscle.
