The PFPTs in my area have not gone into this level of detail. Unfortunately for me, I’ve been seeking help for over 6 years now.

You actually do the physical diagnosis of. medicine. Good for you, most modern day doctors do only computer physicals and barely bother to look at you.

I went for an initial pelvic floor assessment today and she only felt around my hip flexors while I was fully clothed. She then did an internal exam. Would you say a comprehensive assessment should involve an external exam like this? My issues include dull pain in the pubic area which I often feel up into my penis, periodic cramping in the perineum/deep below the balls, periodic sharp cramping pain deep in my bum when I ejaculate, and periodic pain in one testicle. Thank you for the thorough video Dr Susie. I’m located in New York and I have to say unfortunately the in network pfts in NY are not as thorough. It seems to me that in my experience they do not have the comfort level of working with males that you show in your videos. I believe this is such an important aspect to pft because in creating a more comfortable environment I believe the results would be better for the patient as this causes less stress and anxiety. I wish that you were located in NY as I believe if you were my therapist the results would be greater than I am experiencing now. I believe that PFT is revolutionary if done by the right therapists who are comfortable working with males.

Wow finally finished the video. Alot of big words. I need to break out my medical encyclopedia or Thasorouse. I always found it interesting when a examination takes place and the question is asked does this hurt or do you feel pain? At the time usually I am so numb from adrenaline. And #2 I say it doesn’t hurt now but when I go home and things are relaxed I will feel pain. 3) I have been asked show me where it hurts. Even one time from a boxing/sports physician (had to get tested/certified) the Doc asked that and I said I can show you with consent and put her hand exactly on the epididymis of my right testicle.

I still haven’t finished the video in it’s entirety. However as I’m sure you all know I enjoy engaging and keeping up with the thread. As a common theme I read about Dr. G’s exam and assessment video being more involved than others have gotten including myself…My thought is that just because my past exams are not as involved doesn’t mean I hadn’t gotten an adequate elevation, assessment and exam tailored to my needs and concerns. That is just me and I know not everyone’s recovery And process is the same not to mention issues and pain levels. Talk soon.!!!

Examinations should always be tailored to the individual based on their concerns, consent, and clinical reasoning skills. As a clinician, it's important, to ask what is the rationale, intention and impact of said assessment, question, etc. when working with another person.

Hey Doctor Susie, Your male pelvic floor exam was so thorough, I'm shocked. It's a mystery to me how I could go over 25 years never having an exam like this, out of all who have examined me doctors, nurses and specialist have not done a complete tissue mobilizations around the pelvic floor. I was most interested in the scrotal tissue mobilizations and testicular exam going up the spermatic cord into the inguinal canal because my deep scar from my 5 hernia surgeries goes a crossed that inguinal canal at a downward angle to the pubic bone almost. These 3 areas for me are the center of my pain and discomfort and seem to cause pain everywhere else. I can feel tension like stretching in the spermatic cord from the scar tissue to the testicle and no medical personal have done ¼ of that exam on me. I watch this twice and I just wanted to Thank You, that was most enlightening. Rex

Excellent video and again kudos to your model. Now I would be interested in knowing how much of the genital exam I could handle without having to call for time out. Certainly not as much as you present in the video. I feel I'm evolving, but I don't think I would be comfortable having an erection. I did not have a genital exam because I did not present with genital pain or issues. Other than that I don't know what all was included in the exam as I was pretty freaked out trying to deal with my inhibitions and fear of becoming aroused. My wife was present during the exam and I'd like to get her take.

I am at my sister's house getting it ready for sale...with no internet and a weak cell reception...so I haven't view the entire video. Unfortunately it isn't a video you can just view at Starbucks but probably should be. What I have seen tells me the most comprehensive of my three PFPT consulations was not nearly as thorough as what Dr. Susie presented. I actually had an appointment scheduled with Dr. Susie this week and cancelled it thinking I had found a good local PFPT that is somewhat local. Needless to say, I will be rescheduling my appointment with Dr. Susie even though it is almost a six hour drive. I hope that suffices as a positive statement regarding the information given and presentation of the video.

Great video Dr. G.! I just got home from my PFPT where I was told not to mention the word penis or I would not be treated any more. WHAT THE HELL! It's not like I can go out and find another PFPT in my area that treats males and I can't drive 50-100 miles to get to one. So I'll keep my mouth shut and except what I get

The assessment om my pelvic floor exam went nothing like yours. The therapist went nowhere near my penis or testicles. She only did an internal exam and did not go near my prostate. My main concern was an ache at the base of my penis when erect and delayed ejaculation. Thank you for such a detailed examination. We need more pelvic floor experts like you!

I've had 2 PFPTs and neither exam was anything more than palpating my abdomen and then moving to and internal exam. I feel like I wasted years of treatment by not getting a thorough exam.

I haven’t had an assessment like this. Clothed, a therapist could feel me doing a Kegel. The sharp/dull touch test I did. Big revelation is the relief of sensation in the testicular area by eliminating rotisserie store bought chicken. A lot of checks you mention during this exam is totally void in my assessment.

Thanks Doc, this was very instructive. I have been going through a lot of pelvic upheaval ever since having a umbilical hernia repair. Having a enlarged prostate was always the reason I was given. Since a Rezum™ procedure I have come to realize that there's more to the story. I have on my own been working to strengthen my core and work with posture and how I use my muscles and relearn how to poop and pee. Yours is the info I've seen out here that's not squeamish about this part of the male anatomy. My urologist has never given any credence to prostate massage or any other therapy other than MRIs and biopsy to assess and work on what might not be working right. Seems to me that what you just showed me would be so much more informative. I'd jump at the chance to have such a non-sexual evaluation of that area of my body. I am visiting NC next week unfortunately two hours away from your location. ;-( I would otherwise make an appointment to have you or your team for an examination. Meanwhile, I will learn as much as I can from you and any other resources my research finds. If you know anyone in the NY/NJ area, please tell me. Cheers, Bruce

THIS was the educational and informative video I hoped for. I learned alot. My former PT was not as "penis friendly" and our client-therapist relationship ended due to my pyronies never been addressed after 15 sessions. If there is a client that also deals with hydrocele issue I would love to see that video! Great job Doc and ty.
