So the thing about the grand inquisitor is that it has been confirmed that he was one of the guards that were there when Barriss confessed to the senate. Because of her confession he helped the emperor in order 66 killing other temple guards. He did face Vader but obviously he didn't win. As for how he's back he isn't from what I understand is that Vader has bonded the inquisitor spirit to a specific space. (Not sure if it's entirely true. So take that with a grain of salt.)
