With Luffy at least, punch a hole into a rubber eraser or something and you can see why.
Rubber expands a tiny hole into a big one but shrinks back down to a tiny hole after whatever is stabbing through it is expelled.
Also Luffys is rubber which will move and adjust to whatever is being pushed into it. So his bones and organs were pushed around and then went back into position after the hook left his body, not stabbed through.

at the end of the day you have to remember that no matter how deep and amazing most aspects of One Piece are, there will be parts of it that are always going to be present as a byproduct of it being a Shonen manga targeting an audience of young teenage boys. So things like main characters not dying and stupid gags like the Sanji pervert gag are going to be baked into these kind of stories and you will find these same narrative issues with most Shonen manga because its the product of a cultural difference.
