Playtest survey

I personally participated in a lot of playtesting sessions where I live and I thought it could help me improve the game, the experience. Making a game is nice, but I'm not doing it exclusively for me and sometimes I miss obvious problems, issues.
So I tried to use the same system here, trying to remember all the surveys I filled in various game studios.
The whole survey will take you 3 or 5 minutes, maybe 10 if you have a lot to say and share ! You can pass any question you want.
