Many many thanks for the support, I finally conveyed what I wanted for this second part of Emma’s first time giving birth through her nipples. It turned out very intense for her, even more so than her first deviant adventures with insects.
Attracted by her smelly breasts, flies and cockroaches start feasting on her cheese breast milk, they get horny and start penetrating her nipples.

What do you think about insects that can pierce the skin and inject some sort of venom in the tender breasts? I just imagined a wasp stinging the tit. Or maybe a wasp stinging a lobule from the inside and injecting venom. Or maybe a wasp stinging a lobule from the OUTside (through the tit flesh). Or maybe a wasp stinging an undeveloped mammary gland causing an extra breast to start developing.
