One of the things that annoys me about this episode is how clueless Jotaro is, like you said he couldn't recognize Polnareff in the slightest or even be a little bit suspicious of the situation? Although I think it's one of those manga adaptation issues where time moves differently in manga than anime. and yeah this episode made a lot of people uncomfortable because of the child nudity. I haven't seen the Toonami version of this episode but I imagine they probably censored it with a blur or something.
