You were wondering who was playing the piano, it was actually I think 10 different pianists who worked together to make the music. They would have needed to make their own arrangements as well because each time the music played it was different from how the original piece was composed. Each time the characters mention that Kousei's playing changed, was literally the arrangement being changed. So technically the same song, but written differently. I'm not sure how many violinists they had, but there were probably a few who did the same thing for Kaori's playing. Even Aiza's and Emi's playing were different from how the original pieces were performed, so they worked hard to make new arrangements for them as well. There was definitely a lot of love and care that went into the musical scores in this anime. They went above and beyond what typically goes into it.
