Right, clearing up time. Juno was having an affair with Sarah's husband (we all lost something in that crash). That's what distracted him and got him and her bairn killed. The axe got Beth in the neck but missed the the artery and vein while clipping her spine, so it wasn't so much that she killed her it was that she left her alive and paralysed to be eaten. Sarah DID snap completely after having to mercy kill her best friend, with the engraving on the pendant confirming what Juno had been up to. Sarah axed Juno in the leg to hobble her to give her a few extra minutes while the crawlers killed her. Finally, the whole escape was a dream after Sarah knocked herself out (again). The movie ends with her staring at a hallucination of her daughter while the crawlers moved in. Unfortunately the US release chops the end of the movie short.
