I had actually initially planned for this as internal shots, but I also wanted to show Inara's expression, since I think that's part of the appeal of large insertions like this... not simply to show a woman getting something way too big stuffed into her hoo-ha, but also to see someone get completely overwhelmed by such a massive thing.
