If, at this point, you've seen the latest Dr. Strange movie, you'll get the idea that the Darkhold is a Dr. Strange thing from the comics and the movie sorta explains what it is. Also, I get you don't like Radcliffe, but Fitz's "The Doctor" is nothing like Radcliffe. Radcliffe was never intentionally cruel. Misguided, definitely, but it's already been explained why that is. If you remember when we first met him in S3, he just had a mission to improve humanity with transhumanism. The Doctor in the Framework is basically a Nazi.

It is a different reality, just one fabricated by people we know. As far as we know, our reality could also be fabricated. If everything feels real and the consequences are real (maiming, death, etc.), then it's real. Being created with technology AND Darkhold magic makes all the entities in there self-aware. They are "people" and that's their world. We consider ourselves self-aware, but what if someone came to us and said we were all just created and living in a simulation? That they were from the "real world" and we were just programs? We'd probably just think that person was crazy. Same thing to the Framework people. Also, to simply say, "it's not real" or "it's a game" takes away from the severity of it. It's not like a game where there are no consequences, if you're living in there, you feel everything and can die and if you can't leave, that IS your world now. Radcliffe's intention for it was what he said, "a paradise-ever after." It was never meant to be a game. He wanted people to be able to live forever happily. After we met Agnes, you can tell that was his inspiration for the idea. I'm guessing that's what the Darkhold whispered to him.
