The difference in Caroline and Elena fighting is that Elena has nothing to lose. She doesn't care if Caroline lives and frankly she doesn't care if she (Elena) dies. She has no fear. Caroline on the other hand has self preservation instincts and Caroline doesn't want to kill Elena.
