I believe when Sakauchi wanted to curse Matsuyama, he thought to spread it and hopefully becomes viral, so that the chance to kill him greatly increases, rather than him doing it alone. But soon after he committed suicide, he probably didn't know that the curse worked when he was alive, since he was amused and surprised to see so many spirits, cannibalizing even, after he died.(Otherwise he would have definitely stayed alive to witnessed Matsuyama's death, he much less would have known that the school was spiritually closed, since if he had some sort of spiritual powers he would have clung onto life)
I also believe that when Naru decided to reverse the curse, he had already decided to use Hitogatas as proxies, he then put out a theory that the curse would split into 600 and will be less powerful onto each student hence not killing them, but he wasn't sure, his personality was such that he does not want to make promises he cannot fulfill, hence he never once said that the students WILL be safe. He then went on and said that the students must take responsibility for their actions(Which I partly agree, the students did do the action, but they never intended for the death of another, hence they will only receive negative Karma equal to their action and intentions.), and maybe Mai was too focused that the students might die, that she looks at the worst case scenario and think that the students WILL die.
In a kill/save either one situation, Naru has always been trying to save both, I agree with this. We should always look towards a future where both can be saved, and work towards it, when we decide that only one can be saved that is when we have stopped thinking. And if tragedy happens, our intentions has always been to save both, the one who will receive karma is the psycho that created the situation ;)

Not sure if this'll work to hide the relevant spoiler type stuff, but...
I was sure when she walked into that room it was going to be a big ol' mouth.
As for what the kid intended, I don't think he ever considered that he might be as much a victim of the curse as anyone else, even in death.
