In the past episodes it was shown that she befriended that teacher's son when they were in college. She made sure to gauge his personality for years before making him part of her plan to ruin his own father. She knew what he was willing to do for his reputation, especially when she had heard he was becoming a father himself. There's some poetic justice in that punishmen. If you remember the scene where he slapped her, Dong Eun talks back to him saying "what if it was your son going through what I had gone through?" and he was pissed and started slapping her because he always held his son in high regard and not to be compared to what he thought was a low grade piece of shit like Dong Eun. How interesting that in the end, it was his son that killed him.

btw the doctor was close to the patient who went into shock/passed away and he had been denying him coffee for the longest time (mentioned in passing in previous episodes. probably for medical reasons), so when he finally got to bring him coffee he had already passed away. Thats why he was running with the coffee cup and had it spilled all over him ;(((
