Korea is a society extremely concerned with hierarchy & reputation ("saving face"). Remember EAW ep3 "Pengsoo" when those parents were more concerned about their maintaining their dead older son's reputation as a good student? That's not an exaggeration, but more typical in Korea. Things won't "get fixed" because everyone just accepts things as they are and worry more about preserving their reputation. The more higher up someone is, the more important it's gonna be to cover up stuff to preserve their reputation (plus their subordinates have no power against them - hierarchy).

Colonel Seo in the front passenger seat kept asking him "should I give order to shoot" but he ignored her, hinting at her to make the call herself because if things go wrong she'd take the blame instead of him. Then calls her an incompetent bitch for not ordering the shoot earlier on bc now matters only got bigger that they have to deal with while he tries to look like the good guy. Another portrayal of the hierarchy play.
