There, during the chaos of the ceremony, they transferred the babies to something that couldn't die, if we remember correctly, Caroline was the only salvation in that sense, obviously because she was also the only woman.

Also, I know with how the originals timeline with Hope was progressing at this time they already had plans for the future of the tvdu. So, I think finding a way to have the twins still be born was something they had planned and her being pregnant in real life made it easier.
