With regards to the Two Stream facility - I got the impression from the mixed timelines they showed and the greeting from the holographic receptionist which noted that Amy was travelling alone that many of the residents are there in groups - probably multiple family members or friends - the emtire planet is under quarantine and the Doctor's response suggests it hits fast so likely multiple people in a family group would get sick at the same time.

For the most part I agree with your assessment of the Doctor and his team - but a small word in their defence : in between our glimpses of their lives they have other travels, many of which presumably are less eventful and/or less dangerous, hence being relaxed in their travels a lot of the time and not always on their guard. - similarly, we don't often get to see Rory and Amy on their own being intimate or just chilling and hanging out with each other- which can give a slightly off kilter view of their relationship

After the opening scene: ,'She's always waiting. He's always leaving her' - crux of the episode summed up at the start :P

My very favorite Karen Gillan episode. She really is amazing as both old and young Amy. And her plead for Rory’s life to her older self is something I don’t think I’ll ever forget:
AMY: You know when sometimes you meet someone so beautiful. And then you actually talk to them, and five minutes later they're as dull as a brick? Then there's other people, and you meet them and think, not bad, they're okay. And then you get to know them, and their face just sort of becomes them, like their personality's written all over it. And they just turn into something so beautiful.
BOTH AMYS: Rory's the most beautiful man I've ever met.

DOCTOR: Apalapucia.
AMY: Say it again?
DOCTOR: Apalapucia.
AMY Apalapu
DOCTOR: Chia.
RORY: Apalapucia.
DOCTOR: Apalapucia.
AMY: Apalapucia. What a beautiful word.

My favorite of the season. So good. Has some seams, but I just love it so much. Way back when this episode aired, it was what finally sold me on Rory and Amy's relationship after a season and a half. Whenever I revisit them now, I think fondly of them and their relationship, but Moffat really loved sowing doubt in that.

Yes, the last 36 years had been awful for Amy, but they'd made her who she was at that moment. Also, she was only about 57 or 58, and would potentially have had three or more decades of life left to find some happiness. I can understand her fighting for that chance.

I've just noticed that Amy says "No, please, I hate needles". I always thought the same as you, that she said "No, please, I hate you!"
