I definitely headcanon that he‘s only able to climax from internal stimulation (due to the mental gymnastics of being inmate with someone but also the heavy scarring/trauma on his body.) And I picture that the first time he experiences that with someone he finally trusts he’s still a shaking mess the whole time.
