About the historical "adoptions" of heirs, remember it was generally either people from the extended family (Caesar adopted his nephew so he'd become his heir, other emperor often adopted inside their own family) or a guy that will marry someone from your family (it oftens happened in Japan as you say, but it was generally for families without male heir and you had to marry the heiress, generally the daughter of the family chief). So while indeed some cultures had a more "flexible" approach to family, most of the time you still had that bloodline aspect nonetheless.
