A penetrating wound of the abdomen is bad news for us, but with antibiotics to treat the peritonitis it's not a death sentence. Before penicillin, before sulfa, it pretty much was. In a world where saving a life is less important, and a dead patient is just food for other patients, maybe they don't have the drive to research how to treat infection.
