That’s incorrect. Doctors 1-4 were only billed as Doctor Who in the credits, but they weren’t called Doctor Who in the show. They were always just called “The Doctor” in canon since the very first episode

It depends on which incarnation you are watching. it is Doctor Who from William Hartnell (1st Doctor) up to Tom Baker (4th Doctor) they were named Doctor Who. However when Tom Baker left and Peter Davison took over the role the name was changed to "The Doctor. "
