I'm looking at getting a scarification tattoo, which is really hard to get because you're actually have to use the equivalent of dental levers to raise layers of skin, let the scar tissue slowly heal and then do the ink work. Most tattoo artists can't do it because you have to get multiple doctors degrees and letters of approval from the state you operate in.
