Because filming is essentially a live stage show with cameras, they were not able to film the same actor at two different locations at the same time. The best they could do is find someone that looked like the Doctor, and only have the "real" actor playing the double during super close-up shots. They could kind of get away with it a bit better back when TV's were low resolution and stuff like that did not stick out quite so much. The same goes for wires on bats etc.
