I don't think Triss walked all the way to Kaer Morhen. She probably portalled reasonable close by, but far enough away so that if she was tracked it wouldn't be 100% certain where she was headed. As for portalling back to the Brotherhood, I don't think it's a secret that mages operate there, so I doubt its a big deal. How Rience found Kaer Morhen is less clear. He knew Geralt might go there because Jaskier slipped up during the interrogation. As for how he found it's specific location, I dunno. It's a minor detail that doesn't really matter to me. He knew someone that knew, or he figured it out somehow, off screen. It's fine. Edit: Oh, and Rience was spying at Kaer Morhen. He heard Vesimir say that Garalt went to see Nenneke, so that how he knew where to go to find Ciri.

But I don't understand how the fire guy found Kaer Morhen and also why Triss portalled to the mages Brotherhood if portals can be tracked. Honestly, I'm just as confused as you. Maybe it's just plot convenience or something.
