People who suffer from the disease lose their most recent memories first, and often feel like they are literally living far in the past. They may not remember people they've known for decades, including family members. However, there can be moments where the person is lucid and aware and seems to remember events or people they had previously forgotten (which we see briefly at the end of the episode). We can see Beatrice's memory revert, quite possibly back to even when she was a child. She doesn't seem to see the baby as Bojack - she seems to see the baby as her childhood dolly. And yet, at other times she believes she is her young adult self, with her live-in maid who was probably around the house more than her own husband (which could explain why she has more memories involving Henrietta). Notice that she never seems to wonder where Butterscotch is (in her current state of mind, probably assuming he's in his study working on his novel, but also, being as they hated each other, she probably didn't seek him out very often even in their own home). Having worked with people who had the disease, they are often scared and confused, even with the best care (and those emotions often present themselves as anger). It doesn't register with them that they're in their 80's and no, their (now grown) children from 60 years ago don't need to be picked up from school. Some people don't even remember their own grandkids they've had for 30+ years. They can even forget that people have died (that's a tough one to try and explain over and over). So for Beatrice to not remember Bojack? Or at least, to not know his face? Absolutely realistic. It's an extremely difficult thing for everyone involved to go through. Her core memories and traumas are going to be the most prevalent in her mind, especially since she seems to have never gotten any professional help to deal with them. She treated Bojack poorly, due in large part to generational trauma, passing her pain onto him (which, as you said, is not a good excuse). All in all, I think this is really good representation for the struggles of those with Alzheimer's.
