I'll give what I understand from the scene as someone who has read the novel. Wei Qian is actually afraid of the complication that could arise from the surgery, of becoming some sort of an invalid as a result, and thus becoming a burden to his family like his mother was to him before she died. So he is not only afraid of becoming an emotional burden in case he died but also a physical burden in case he lived but with side effects.

2024-05-09 08:25:49 +0000 UTCMy personal understanding is when you suffer in life for so long you tend to resign yourself to the fact that it's your fate to always suffer in life. It's hard to imagine a positive future ahead. I feel like that was Qian's mindset also throughout the whole drama. That's why he never wanted anything for himself or planned for the future. When he got the news about the complications from his condition, he automatically resigned himself that he would suffer the worst case and die according to his fate. And by him dying he would drag his family down into despair also. Like his mother dragged him into. It's clear Qian has mental health issues he needs to overcome and mental health overpowers medical logic 99% of the time. That's why he needs someone by his side to provide a hand and pull him back up when he's down. EDIT: Also, thank you for your wonderful reactions.
