He is a sleep paralysis demon that takes form of a being that the sleeper desires to fuck the most. You know it's them due to their eyes having a red spiral in them.
The form he is taking here is what Jamie (main character from the comic) wants to fuck. a short goblin-like creature.
I drew it with a massive cock and balls because I wanted to. :)
