The way I understood the mv was that he's in the coma and while he's in the coma, he's dreaming that he keeps trying to get to his girl. The girl is a very young Suzy from miss A. It had a tiny bit of controversy because I think Suzy was about 18 at the time and Son Ho Young was in his 30s. But glad you guys liked it :)
