How I interpreted Carta's "McGillis" scene is that, on the verge of death, she was praying and calling for help from McGillis, so when "McGillis" (Gaelio) came and rescued her, her delusions and prayers were "answered" as she was already drifting off into death, so she couldn't really differentiate between Gaelio and McGillis at that moment. And it was the fact that she couldn't even recognize Gaelio, one of her longest friends from childhood, that made Gaelio cry as he assumed the (temporary) identity of McGillis to ease Carta's suffering albeit briefly.
