Oh yeah, since it might have slipped your mind, that sword we saw (briefly) that was piercing Dragon Tohru, was mentioned last episode during the end credits as Tohru awoke from a nightmare where she was pierced by it, that it hurt... and as said here, we know she escaped to this world and let people think she was dead, so that should answer your confusion about the sword you seemingly had! ^^

It'll be harder to identify with Tohru, since her color scheme in dragon form is green, but whenever you seen anyone with a green-yellowish transparent hue around them (like Tohru & Kanna in this episode), that means they have "perception blocking" casted upon them blocks anyone from noticing them, as was explained, and when they do not have it around them (like Kanna and Tohru peeking above the clouds), you can assume it's no longer active and they're visible if you look where they are... just a good mental note to make for the future, my guy!

Maid Dragon's a special enigma that continuously builds up more craziness and stacks character development upon character development all throughout its course that makes the characters just feel authentic, realistic, and very sympathetic after seeing them first, watching them grow, learning their origins, and how they come out better than before because of these little interactions between dragons and humans... plus the animation's always super high quality for dragon-related scenes (whether fighting, transforming, or comedic gags) that you can't find as consistently in other shows very often!
