Shigechi's head (and the rest of him) is what I think is just "unique character design". I think it sets up a stereotype like if you read a story without images and it describes how Shigechi behaves, how Shigechi looks like is from our perspective and because of that we would imagine someone who did all the stuff he did would be someone small, deformed and snotty. So that is why I call it a stereotype, because he is what we expect him to be.
