Not only she has a small bladder, but she also has, canonically, suffered of incontinence due to physical and psychological conditions. Even as an adult, she sometimes struggles when it comes to have to go to the bathroom, especially when she denies her own need and waiting until the very end only to prove that she can hold it - even if sometimes it doesn't work out too well...
