With the laughing girl, her name is Toko, but the same way that Robin and Nami to fit in are called Orobi and Onami (as well as Tsuru and Kiku as Otsuru and Okiku), you end up calling Toko instead Otoko, which is the term for "male." So the play on words is that her name is Toko, but when you say her name properly as Otoko it's like you are calling her "male" as her name.

I think you're confusing oirans and geisha. oirans are sex workers but they do get to choose who they sleep around with. I think I saw a documentary once where they mentioned that socially, oirans are above normal people when it comes to social status which is why they're picky when it comes to their customers. geisha are the entertainers who do not offer sexual services, they're what you mentioned as being a paid company. also japanese prostitution ≠ western prostitution, meaning a woman and a man drinking and eating together can be considered a form of prostitution if they don’t know each other (in that time period, specifically)
