Everyone in the school believes "Fitz" to be male. I doubt they will go to her point of view but she does struggle with this and wants to tell him but doesn't want to betray Ariel's trust in her.

Also keep in mind that in the world of Mushoku, there's Detoxification magic (aka status heals). Just heal that herpes with magic. I don't know what level you have to cast to cure STDs, but it's out there.
