Not only that, but he hasn't slept in at least thirty hours, if not more, and he spent the majority of that time undergoing a brutal form of torture - twenty hours, in addition to the time that he was under the effects of the poison. Moreover, his body was forcibly aged by ten years over that twenty hour period, where he underwent extreme starvation. Ivankov's method of healing him involved mass and rapid cell regeneration to boost his immune system to the point that it could fight off the toxins; technically speaking, his body is effectively now twenty seven. Most of the rest of that time has been spent fighting for his life in a hellscape. This, all before entering another form of hell, taking the form of a war between two superpowers, while missing the parts of himself that his crew holds. Luffy's not in a good state at all here, physically, mentally, or emotionally.
