I always interpreted the “I’ll seriously consider it when you’re 16” as just him trying to give her motivation to live through cancer and have something to look forward to because I could never read anything into it other than a siblings dynamic tbh. It helps with making it far less creepy.

Gorou saw Serena like his little sister, where as Serena was the young, doting little sister that wanted to marry her brother. Also, the ending credits changed a little for this episode.
