This episode breaks my heart! To see how many bad thoughts Sam has about himself, and his fears, then having these fears come to life at the end. With him hallucinating Dean calling him a monster, then eventually Dean actually calling him a monster. Then having Dean use John's words from when Sam went off to College. Or at least similar. That definitely didn't help their already terrible situation. It's all so horrible. And the scene he had with Bobby! How he basically told Bobby the only way to help him was to shoot him. Part of him wants to die. And after what he just went through, I can't blame him. But he's felt that way since at least Dean went to Hell... And that's part of the problem tbh. Killing Lilith, to him, was always a suicide mission. He didn't have Dean anymore, so he was determined to kill Lilith and die doing so. Then Ruby came along and introduced him to all this stuff, Demon Blood and his powers, and he probably thought there was no reason not to as he was planning to die anyway. Then Dean came back, and he was already addicted to Demon Blood and so he wasn't gonna tell Dean about it because it's an addiction and he's not gonna risk being forced to stop or making Dean look at him like a monster. But before Dean knew about it, I think it partly stopped being a suicide mission as he wanted to live for Dean. But then Dean found out and he took it badly, and he betrayed him (in Sam's mind) by locking him up and forcing a detox that was very painful and traumatising for Sam, and so I think it almost became a suicide mission again. Or at least him really not caring whether or not he died. But we know it started as a suicide mission when Dean was in Hell, Ruby was trying to stop him from getting himself killed while going after Lilith. So it doesn't seem far fetched that it'd go back to that once he felt he no longer had Dean. :(
