How about this as hypothetical cause to effect series of events. Rather dreaming about the person and them miraculously becoming a patient of his. Patient comes to this hospital for treatment because of the evident connection of his uncle and Cuddy. House inadvertently sees him and the historical but unfamiliar recognition triggers a dream. The unsolvable nature of the case is what gets House and his team the referral after the fact. Merely a case of occam's razor thinking, rather than dreaming a situation into existence. Just a thought.
