A lot of people seem to think that the first time but that dude is not Josuke from the future. Remember Koichi is the one telling the story and he has no idea what the guy looks like, only just that he looks like Josuke. Josuke's hairstyle was also very common with delinquents that's why Rohan calls it old. Araki explained in an interview that the point of the flashback was to show the golden heart of Morioh. That some dude obviously in his own situation still went out of his way to help a mother and her son. Thematically it fits perfect cause here we see Josuke was struggling awakening his stand cause of Dio and that act of kindness inspired him and gave him the mental fortitude to awaken and control his stand unlike Holly. Josuke then goes on to awaken the kindest of all stands, the ability to save others but not himself. Just like that man who saved him all those years ago who could save others but not help himself.
