I ay its negative self image, he's unable to see himself as the cheerful, talented, intelligent young man he is now. He just sees that thing he was that he hates so much. He can't see himself as Rudy, I think on some level he really still thinks this is some fever dream as he lays dying on a operating table. That my opinion
