The field of flowers and the river is the entrance to the land of the dead in Japanese Buddhism. The river is called Sanzu, and you cross it on a bridge, ideally (you have to pay a fee). I agree, this is probably the first relly completely serious moment for Luffy that we see. And that the situation is really tense and intense without having to resort to simply be about if someone is going to die or not.
