Go back and watch the first time Damon thinks he is being rescued by Elena, how soft the lighting is. Then look at the difference in how the lighting is shot and the focus is sharper when he is actually saved. This is used quite a bit in the show and will help foreshadow and understand what is a dream/vision and reality.
