With regards to what happened at the end, I think Erza's first throw of the kunais hit all the boys. When she realized it was them she was perfectly willing to invite them in, but the other girls didn't want that. I don't think Erza attacked the boys off-screen. She always says she doesn't mind bathing with them and has done it in the past.

The scenes in the Hot Spring are from the manga and the scene of Lucy going to Natsu's house is from a short that Hiro Mashima wrote. The next OVA isn't took far away from being watched. OVA 5 should be watched between episodes 170 and 171. It is adapted from a chapter in the manga that also gets skipped in the main anime.
