Alluka being trans is the most logical conclusion to me, at least; but because Togashi decided to be subtle and communicate all of this strictly through Zoldyck family dynamics, some people like to think that Alluka is biologically female and the family is referring to her as "him" as a way of distancing her from the family. Kind of like referring to her as "it", in a way. Then there are the people that DO believe all Zoldyck children are male but don't like the idea of transgenderism so they seem to resort to shouting "la la la" every time Alluka is referred to as female and try to pretend "NOTHING IS GOING ON HERE!"

But that could be from how I was raised in a military family to show respect until a person has not earned my respect anymore. And fair point minashiro on togashi being a fucking troll
