It's weird that almost everyone is confused by this. Yes, the scene itself is a little vague, but in the little x-ray shot where Tsukasa is describing "I'm going to sever your cervical nerve", it's pretty clear by the sound effect and visuals he means "by snapping your neck", not cutting it.
