You’re overthinking it. 16 was the legal minimum age at which a female could enter into marriage in Japan (it was moved up to 18 recently). He’s not saying he’ll consider someone to be a romantic interest at age 16. It’s a straight faced joke

Seems like Caleb & Jacob were weirded out by the doctor/Goro a bit before Ai arrived at the hospital. Just wanted to provide some bits that may help it feel better.
The hospital scene with Sarina, she says she wants a beautiful face when she’s reborn in her next life. Goro shoots down reincarnation as a dream and this obviously hurts her feelings as she’s dying. He doesn’t play into her fantasy. Then later when she asks him to marry her. He decides not to have callous bedside manner and he plays into her fantasy. Saying he’ll think about it when she turns 16 (it feels weird but that’s the league age for a number of things in Japan, I’m not saying it’s right/wrong). It’s his way of saying no while not stomping all of her fantasy like he did before. They both know she’s close to dying and won’t make it to 16. While it’s a flawed way of trying to console his patient, his heart is in the right place, it’s just awkward.
- I also think Akasaka went this route to say something about the perception around idol fandom/culture. A lot of people who are outside the fandom look at the legion of male fans that idols attract and there’s a stereotype around them. That idol otaku are NEETs, social inept, fat, unattractive, not successful, uneducated. And that a lot of these descriptors lead us to thinking they’re creepy or untrustworthy. Goro is seen from the outside as being the opposite. He’s a young, well-educated, good-looking, successful doctor. He’s basically achieved the dream of most Asian parents want for their kids. But even the lies the idol industry as a whole and everyone (from producer to idol and everywhere in between) work on people even like this. We expect creepy idol fan behavior from guys who look like the stalker from Perfect Blue. There’s been countless studies in the last 20-30 years that have correlated attractiveness & success with a person being considered more trustworthy or deserving of praise (in a vacuum).

Fun little bit of trivia that gets lost in translation: after Aqua and Ruby put on the god act and confront Miyako, from that point on she addresses them with a formal "keigo" style of speech as if they are her superiors, which is incredibly weird-sounding to Japanese speakers. It's like calling babies "Sir" or "Madam" or something. Even after she offers to become their mother, she's still speaking that way. I'll leave it to your imagination regarding whether or not she changes the way she addresses them after living with them for 15 years and watching them grow up as their mother, but you can probably guess.
